(一)皈依

南無古魯洛格學(日阿)雅 敬禮觀音大悲尊

諸佛正法賢聖三寶尊 從今直至菩提永皈依

我以所修施等諸資糧 為利有情故願大覺成

(二)發心

A﹒四無量心(此乃生起菩提心之基，故於發菩提心之前，先修四無量心。) （三遍）

願諸眾生 永具安樂及安樂因

願諸眾生 永離眾苦及眾苦因

願諸眾生 永具無苦之樂我心怡悅 於諸眾生

遠離貪瞋之心住平等捨

B﹒發菩提心(為速登佛位，故必發大菩提心，方進入大乘,而穩速證道。)（三遍）

珍寶人身難得今已得 甚深佛法難聞今已聞

爲使自他皆獲殊勝益 願我速發無上菩提心

往昔善逝如何發菩提 以及次第行菩薩學處

我亦如是發心為利眾 如彼次第學修菩薩行

我今發心不為自求人天福報

聲聞緣覺乃至權乘諸位菩薩

唯依最上乘發菩提心

願與法界眾生

一時同證阿耨多羅三藐三菩提

一切輪迴傍生如母眾 皆因無明愚癡增上力

為引彼等解脫聖道故 緣起放生儀軌勝義印

如教行持必定無欺惑 仰賴三寶加持得解脫

如是念誦並思維彼等有情已得三寶之救護，並接誦：

頂禮供養皈依出有壞、善逝、應供、

正等覺寶髻佛。（三遍）
頂禮供養皈依出有壞、善逝、應供、

正等覺本師釋迦牟尼佛。（三遍）
頂禮供養皈依出有壞、善逝、應供、

正等覺怙主阿彌陀佛。（三遍）
頂禮供養皈依大慈大悲觀世音菩薩。（三遍）

虔誦阿彌陀佛心咒:

嗡 阿彌德瓦 阿依斯底吽啥

虔誦觀世音菩薩心咒：

嗡嘛呢唄麥吽啥

接誦甚深緣起偈：
南無！

諸法由因生，彼因善逝說，
何遮彼法因？佛陀如是說：
諸惡莫做，眾善奉行。
自淨其意，是諸佛教。

此後誦長壽佛心咒：

嗡阿嘛惹尼遮溫帝耶娑訶

觀想此等有情從生命畏懼中解脫，生生世世獲得壽命悉地。施放丸子等加持品後，接誦：

嗡桑巴瓦速達 沙瓦達麻 桑巴瓦速多杭

從空之中，自己面前種字變成廣大寶器，三字安立，嗡字出生無礙無漏甘露大海，量等三千成。嗡阿吽(三遍)

觀想諸法變空性．與空性中所緣眾生之身蘊界處等，悉皆變為清淨大樂自發之不可思議供雲。

再以變食真言作加持：

南無薩瓦達他嘎答 阿瓦洛格得 嗡桑巴(日阿)桑巴(日阿)吽(三遍)
所有十方刹土中，所住佛法及僧伽，
上師本尊空行眾，如海守誓護法前。
奉獻殊妙供品物，具有命氣心識者，
此等眾生解壽魔，獲得無畏之解脫！
於此一切蘊處界，自性清淨供養雲，
周遍廣大虛空界，普具妙欲大安樂，
成就無死延壽物，顯現無邊無垠故，
酬謝汝等之意願，願諸具足大歡喜！
吾與一切諸眾生，願解非時橫死災，
長久住於此世間，獲得圓滿諸吉祥！
祈願恢復衰界蘊，增上動靜之精華，
壽得光華及威嚴，無病法財皆增上！
此善回向諸有情，願得殊勝金剛身，
超離一切生死位，願獲無量光佛果！

若為祈請上師住世而放生者，則當念誦：

行此善妙緣起力，祈願具德上師尊，
住世事業遍十方，成就一切所欲義。

接誦祈願：

霍！以舍輪回劣三門，及播解脫種子力，
祈佛法盛眾生樂，普除一切惡趣苦。

願能無礙速現前，殊勝解脫一切智，
捨棄不淨此身時，願獲極樂刹化身！
生已刹那行十地，化身饒益十方眾，
願諸眾生具安樂，願諸惡趣恒成空！
所有住地菩薩眾，願彼究竟皆成就！
普除害他習氣力，夢中亦不起惡念，
願恒不離菩提心，清淨戒律增吉祥！

願我能以放生此善業 速捷成就上師佛果位

並將一切沉溺諸眾生 善巧導引安立於彼地

勝妙菩提心寶極珍貴 祈願諸未生者令生起

更願令已生者不衰退 輾轉增上至結菩提果

惟願我及諸佛菩薩三世累積的一切功德,讓被放生的傍生道如母有情永不再墮三惡道，能立刻往生阿彌陀佛國土或獲得暇滿人身，迅速成就無上正覺；讓漁夫、獵人和一切殺害動物者即時深信因果、皈依三寶,能夠遵循寂靜的正道，且速成正等正覺！

惟願我及諸佛菩薩三世累積的一切功德,讓上師善知識及一切利他的聖者常久住世，廣轉法輪,利益一切如母有情！

惟願我及諸佛菩薩三世累積的一切功德,讓一切住持佛法的僧眾及護持佛法的居士僧伽長壽，速即成就無礙智，圓滿明了證悟！

惟願我及諸佛菩薩三世累積的一切功德,成為一切有情安樂的源泉，也是唯一能治療眾生痛苦的良藥——佛法在一切有情心中增長、不衰退！

惟願我及諸佛菩薩三世累積的一切功德,讓諸施主皆能恆長(包括協助今天放生的人)護持正法，健康長壽，凡所希冀如法成就！

惟願我及諸佛菩薩三世累積的一切功德,讓一切患病如母眾，尤其是患癌症、愛滋病、長期昏迷症等一切可治或不可治之疾病者，立刻脫離疾病，即時獲得長壽，值遇善知識,聞殊妙佛法，速發菩提心，瞬間證正覺;讓一切持戒、利他、皈依三寶、過有意義生活的人都能長壽,速成正覺！

惟願我及諸佛菩薩三世累積的一切功德,讓我等一切眾生生生世世皆能值遇大乘善友善知識，並將善知識視為佛陀，讓我及諸有情所為皆令善知識歡喜，生生世世得彼慈悲摄受直至證悟菩提！

惟願我及諸佛菩薩三世累積的一切功德,讓我無有絲毫傷害任何一個有情的念頭生起，讓我所有的經驗，無論苦或樂，對如母眾生都有最大的益處！

我回向功德如同三世諸佛回向功德一樣。惟願娑婆教主釋迦牟尼佛圓滿清淨、結合經續的教法在我及六親眷屬、一切弟子眾的心流中成就，尤其是成就菩提心及無上瑜珈密續的大圓滿光明！惟願我等此世能夠證得正覺，並藉清淨佛法的弘揚，引導一切有情獲證無上佛果！

惟願我及一切有情、諸佛菩薩三世所積由心名言假立的功德,是僅僅由心名言假立的我，成就由心名言假立的佛果，並且引導一切亦是僅僅由心名言假立的有情，到達由心名言假立的正等正覺。

Convert to Guru Avalokitesvara

Salute Guanyin great passion Bodhisattva

諸佛正法賢聖三寶尊 從今直至菩提永皈依

In the Buddha, the Dharma and the Supreme Assembly

I take refuge until I attain enlightenment.

我以所修施等諸資糧 為利有情故願大覺成（三遍）

Through the merits of practicing generosity and so on,

May I attain Buddhahood for the benefit of all sentient beings.

願諸眾生　永具安樂及安樂因(慈 無量心)

May all beings have happiness and causes of happiness forever (Kindness immeasurable mind)

願諸眾生　永離眾苦及眾苦因(悲 無量心)

May all beings be free from suffering and causes of suffering forever (Pity immeasurable mind)

願諸眾生　永具無苦之樂我心怡悅(喜 無量心)

May all beings forever enjoy painless, immaculate blissful stage (joy immeasurable mind)

於諸眾生 遠離貪瞋之心住平等捨(捨 無量心)

May all beings be free from craving and anger, rejoice in the well-being of others (Indifference immeasurable mind)

B﹒發菩提心 (為速登佛位，故必發大菩提心，方進入大乘，而穩速證道)

B. Bodhicitta-initiation (in order to achieve Buddhahood quickly one must initiate the great bodhicitta. Then entering the Mahayana and attaining enlightenment steadily)

May I initiate supreme Bodhi-citta immediately.

往昔善逝如何發菩提 以及次第行菩薩學處

How The Well Gone One initiate bodhicitta,

And practice Bodhisattva’s actions in order,

我亦如是發心為利眾 如彼次第學修菩薩行（三遍）

I initiate bodhicitta to benefit beings too,

And follow them practice Bodhisattva’s actions in order. （three times）

為引彼等解脫聖道故 緣起放生儀軌勝義印

For leading them to the path of liberation,

Life-freeing ritual supreme sign arises from such condition.

如教行持必定無欺惑 仰賴三寶加持得解脫

Practicing on the ritual must lead to result without deception,

He will liberate by relying on Ttiratna blessing.

頂禮供養皈依出有壞、善逝、應供、正等覺本師釋迦牟尼佛

Making obeisance to、 offering、 taking refuge in Bhagavat、One Well Gone、One Worthy of Offerings、Samyagbuddhi、Root Guru Sakyamuni Buddha.

頂禮供養皈依出有壞、善逝、應供、正等覺怙主阿彌陀佛

Making obeisance to、 offering、 taking refuge in Bhagavat、One Well Gone、One Worthy of Offerings、Samyagbuddhi、Guardian Master Amitabha.

頂禮供養皈依大慈大悲觀世音菩薩

Making obeisance to、 offering、 taking refuge in great kindness and mercy Gwan Shr Yin Bodhisattva.

虔誦 阿彌陀佛心咒 : 嗡 阿彌德瓦 阿依斯底吽啥

Chanting piously

The mantra of Amitabha: Om amediwa aye seddhe hong sre

虔誦 觀世音菩薩心咒 ： 嗡嘛呢唄麥吽啥

Chanting piously

諸法由因生，彼因善逝說，何遮彼法因？佛陀如是說：

Sarvadharma comes from causes,

The causes are gone as they should go,

What cover the dharma and causes?

It was said by Buddha:

諸惡莫做 眾善奉行 自淨其意 是諸佛教

To do no evil, to do only good, to purify the will, is the doctrine of all Buddhas.

此後誦

Then chanting

長壽佛心咒：嗡阿嘛惹尼遮溫帝耶娑訶

The Longevity Mantra: Om a ma ra ne dze ban te yi so ha

觀想此等有情從生命畏懼中解脫，生生世世獲得壽命悉地。施放丸子等加持品後，接誦：

Meditating that those sentient beings liberate from the fear of life and obtain the life Siddhis forever. Then chanting:

嗡桑巴瓦速達 沙瓦達麻 桑巴瓦速多杭

Om Sumbra Suda Sara Dama Sumbra Sudo Hung

從空之中，自己面前種字變成廣大寶器，三字安立，

From the emptiness, seed character changes into broad treasure implements in front of me. Three characters on there,

嗡字出生無礙無漏甘露大海，量等三千成 嗡 阿 吽(三遍)

Om becomes amrta sea permeating no outflow everywhere,

Quantity are trichiliocosm. Om Ah Hum (three times)

上師本尊空行眾，如海守誓護法前。

Guru 、The original honored one、 Dakinis,

In front of sea-like Dharmapalas and promise keepers,

奉獻殊妙供品物，具有命氣心識者，

To dedicate extraordinary offerings,

ones with vital energy of mind and cognition,

此等眾生解壽魔，獲得無畏之解脫！

They all liberate from the Mrtyumara,

obtaining deliverance without fear!

於此一切蘊處界，自性清淨供養雲，

Everywhere skandha- field – dhatu,

self-nature pure offering cloud,

周遍廣大虛空界，普具妙欲大安樂，

Over the huge emptiness,

Full of subtle desire bliss,

成就無死延壽物，顯現無邊無垠故，

Becoming a long life creature without death,

to show the boundless and infinite result,

酬謝汝等之意願，願諸具足大歡喜！

As a reward for such aspiration,

wishing everyone full of great happiness!

吾與一切諸眾生，願解非時橫死災，

All the beings with me,

May we liberate from accidental death and disaster,

長久住於此世間，獲得圓滿諸吉祥！

Staying in the world for long time,

Obtaining the perfect auspiciousness!

祈願恢復衰界蘊，增上動靜之精華，

Praying for the recovery of declining dhatu-skandha,

Increasing the essence of activity.

壽得光華及威嚴，無病法財皆增上！

Obtaining long life with glory and majesty,

Increasing both Disease-free and dharma-rich.

此善回向諸有情，願得殊勝金剛身，

Such goodness rewards to all the sentient beings,

May they obtain remarkable Vajra body.

超離一切生死畏，願獲無量光佛果！

Separating from all the fear of birth-death,

May they obtain the Buddhahood fruition of infinite light!

To obtain achievements all ones want.

Eliminating the sufferings of all the evil destinies widely。

願能無礙速現前，殊勝解脫一切智，

May unimpeded state happens quickly,

the same as remarkable liberation omniscience,

捨棄不淨此身時，願獲極樂刹化身！

While abandoning such impure body,

May one have emanation body in the ultimate bliss realm!

生已刹那行十地，化身饒益十方眾，

Having been in ten directions at birth,

Emanation body benefits all beings there.

願諸眾生具安樂，願諸惡趣恒成空！

May the beings have peace and happiness,

May the evil destinies become emptiness forever!

所有住地菩薩眾，願彼究竟皆成就！

All the Dwelling-place Bodhisattvas,

May they become absolute Siddhas!

普除害他習氣力，夢中亦不起惡念，

Eliminating the habit force of hurting others widely,

No evil thoughts arising in the dream,

願恒不離菩提心，清淨戒律增吉祥！

May I always be with Bodhi-citta,

The pure rules and more auspiciousness!

Guiding them skillfully to the same realm.

勝妙菩提心寶極珍貴 祈願諸未生者令生起

Marvelous Bodhi-citta is supreme precious,

May the beings have it if they are not getting such treasure.

更願令已生者不衰退 輾轉增上至結菩提果

Further may the beings with it no more decline,

Increasing it repeatedly until they get Bodhi-fruition.

All the merits and virtues accumulated by me, Buddhas and Bodhisattvas in the Trikala, which let Guru Wise Advisor and saints for benefiting others live in the world, to preach Buddha-truth widely, and benefit all the motherlike sentient beings

惟願我及諸佛菩薩三世累積的一切功德，讓一切住持佛法的僧眾及護持佛法的居士僧伽長壽，速即成就無礙智，圓滿明了證悟！

Best wish:

All the merits and virtues accumulated by me, Buddhas and Bodhisattvas in the Trikala, which let all the maintaining-dharma assembly monks and supporting-dharma lay people Sanghas have long life, obtain omniscience of Buddha, become perfect clear enlightment.

惟願我及諸佛菩薩三世累積的一切功德，成為一切有情安樂的源泉，也是唯一能治療眾生痛苦的良藥 — 佛法，在一切有情心中增長、不衰退！

Best wish:

All the merits and virtues accumulated by me, Buddhas and Bodhisattvas in the Trikala, which become the source of peace and happiness to all the sentient beings, and make the unique medicine for curing the sentient beings suffering——dharma, increase in the heart of the sentient beings, without declining!

惟願我及諸佛菩薩三世累積的一切功德，讓諸施主皆能恆長(包括協助今天放生的人)護持正法，健康長壽，凡所希冀如法成就！

Best wish:

All the merits and virtues accumulated by me, Buddhas and Bodhisattvas in the Trikala, which let almsgivers constantly(including people who help to set animals free today)support the correct doctrine of the Buddha. They have long and healthy life, and achieve just what they want from the Dharma.

惟願我及諸佛菩薩三世累積的一切功德，讓一切患病如母眾，尤其是患癌症、愛滋病、長期昏迷症等一切可治或不可治之疾病者，立刻脫離疾病，即時獲得長壽，值遇善知識，聞殊妙佛法，速發菩提心，瞬間證正覺;讓一切持戒、利他、皈依三寶、過有意義生活的人都能長壽，速成正覺！

Best wish:

All the merits and virtues accumulated by me, Buddhas and Bodhisattvas in the Trikala, which let all the sick motherlike sentient beings, especially those suffering from cancer, AIDS, long-term Coma, and other treatable or incurable diseases, get rid of disease immediately, and have a long life at once, meet with one well known, hear supreme dharma, have Bodhi-citta quickly, become completely enlightened instantly; let all the people, who are in morality、benefiting others、taking refuge in triratna、meaningful living, have long life, become perfect universal wisdom quickly!

惟願我及諸佛菩薩三世累積的一切功德，讓我等一切眾生生生世世皆能值遇大乘善友善知識，並將善知識視為佛陀，讓我及諸有情所為皆令善知識歡喜，生生世世得彼慈悲摄受直至證悟菩提！

Best wish:

All the merits and virtues accumulated by me, Buddhas and Bodhisattvas in the Trikala, which let all the beings meet Mahayana friends of virtue and ones well known, regard ones well known as Buddha. Ones well known are happy by the actions of mine and sentient beings. Mercy supervising obtained by us from them until we are completely enlightment in the generation after generation.

惟願我及諸佛菩薩三世累積的一切功德，讓我無有絲毫傷害任何一個有情的念頭生起，讓我所有的經驗，無論苦或樂，對如母眾生都有最大的益處！

我回向功德如同三世諸佛回向功德一樣

Best wish:

All the merits and virtues accumulated by me, Buddhas and Bodhisattvas in the Trikala, which let me have no any thought arising to hurt sentient beings; Let me take all my experiences, whether bitter or happy, benefit motherlike sentient beings best!

My transference achievement is the same as those Buddhas’s in the three periods.

惟願娑婆教主釋迦牟尼佛圓滿清淨、結合經續的教法在我及六親眷屬、一切弟子眾的心流中成就，尤其是成就菩提心及無上瑜珈密續的大圓滿光明！

Best wish:

Buddha Sakyamuni, teaching host in the Saha world, whose perfect pure doctrines of the sutras will become in the continuity-consciousness of all the disciples，especially become Bodhi-citta and great full light of the unsurpassed yoga tantra!

惟願我等此世能夠證得正覺，並藉清淨佛法的弘揚，引導一切有情獲證無上佛果！

Best wish:

May we become perfect universal wisdom in this life，by spreading the pure Dharma, to lead all the sentient beings obtain the unsurpassed Buddhahood fruition!

All the sentient beings and I 、the merits and virtues accumulated by Buddhas and Bodhisattvas in the Trikala unreally constructed by mind conceptualization，Only through ego unreally constructed by mind conceptualization, become Buddhahood fruition unreally constructed by mind conceptualization, and lead all sentient beings unreally constructed by mind conceptualization to achieve Samyaksambuddha unreally constructed by mind conceptualization.
